Rules of Acknowledgement, Steroid, Concussion, and Emergency Card forms to be completed online before the 2013-2014 school year begins.

2. Go to:  "SPORTS PARTICIPATION PACKET"

FAQ: What if I have signed a paper copy do I still have to do this online?

Answer: Yes you do, this is a new athletic policy and we have to do this online now. 

7th grade

 Physical, Medical history given to Coach Kirk, Acknoledgement of rules, Pink Emergency information card all need to be filled out online

8th grade

 Physical,Medical history given to Coach Kirk, Acknoledgement of rules, Pink Emergency information card all need to be filled out online 

9th grade

Physical, Medical history given to Coach Kirk, Acknoledgement of rules, Pink Emergency information card, UIL Steriod and Concusion sheets all need to be filled out online
