In an abundance of caution, HEB ISD is cancelling all after-school student activities for today, Tuesday, May 21, 2013. All schools will dismiss at the regular time. Buses will run their normal routes at normal times. Documents Needed for PreK Enrollment 2013-2014
q Birth Certificate (child must be 4 yrs of age on/before Sept 1) q Current Immunization Records q Social Security Card(if available) q Proof of District Residency -Current utility bill with parent/guardian name and address (telephone bills will not be accepted) or… -Current lease agreement with parent/guardian name and address or… -Notarized statement of residency q Driver’s License of Parent/Guardian q Proof of Income -Current pay stubs with gross income for all household members and parent/guardian name/address info. (“current” meaning month prior to verification (ex. July 12 – Aug. 12) -Tax Return for 2012(Required for Head Start or SelfEmployed) q Proof of military service for active duty parent(s)(if applicable) q Proof of Foster Care/State Adoption (if applicable) |
